Liability Release Waiver

1. Assumption of Risk: I understand that despite the best efforts of the dog boarding and daycare service to provide a safe and secure environment, the inherent risks of dog care, including but not limited to dog fights, injuries, and transmission of illness, cannot be entirely eliminated. I accept these risks and assume full responsibility for my dog during its stay.
2. Release of Liability: I release, waive, and discharge The Tail Wag Hotel Inc., its employees, agents, and representatives from any and all liability, claims, or demands for injury, illness, death, or damage to myself, my dog, or my property, which may arise during or as a result of my dog's participation in services provided by The Tail Wag Hotel Inc.
3. Veterinary Care Authorization: In the event of illness or injury, I authorize The Tail Wag Hotel Inc. to seek veterinary care for my dog at my expense. I agree that The Tail Wag Hotel Inc. is not liable for the costs of such care or the outcome of any medical treatment.
4. Behavior and Aggression: I certify that I will inform The Tail Wag Hotel Inc. of any known behavioral issues, aggression, or history of biting. If my dog has exhibited any unsafe behaviors, I will disclose this information during registration. I understand that I am solely responsible for any injury, damage, or harm caused by my dog to other animals, staff, or property while in the care of The Tail Wag Hotel Inc. I further agree to cover any associated costs for damages or medical treatment resulting from my dog’s actions.
5. Emergency Contact: I agree to provide emergency contact information and ensure that I or my emergency contact will be available during my dog’s stay.
